PTA Participant Waiver

Every year the Hickory PTA is required to have a PTA Participant Waiver on file for all Hickory students participating in PTA sponsored activities like Winter Fair and Jog-a-thon.
Please complete your student’s waiver here: https://forms.gle/JmNwvSXAk8MATHBu6.
Teachers will also be asked to send the electronic waivers to all parents, you only need to complete it once.
